Ashacre Homes provide responsive placements to support hospital discharge and step‑down pathways.

Referral Criteria:

Adults requiring short‑to‑medium term independent living accommodation
Individuals able to live independently with agreed support
Discharge from hospital or clinical setting
Clear reablement or move‑on plan in place

Ashacre Homes is designed to support social authorities with effective discharge and reablement solutions. Referrals are therefore accepted from local authorities, NHS teams and integrated care systems. All referrals are subject to suitability assessment and availability.
